Purpose of Activity

To present a quality improvement (QI) project implemented in a home-based palliative care program to imbed the Edmonton Symptom Assessment System (ESAS) into the electronic health record and to then track symptom severity and collect utilization data.

After completing this continuing education activity you will be able to:

  1. Recognize the benefits of and recommendations for home-based primary and palliative care programs and the methodology of the QI project.
  2. Summarize the results of the QI project implemented in a home-based palliative care program.
