Purpose of Activity

To provide information on health coaching to patients with a primary or secondary diagnosis of Type 2 diabetes mellitus to increase self-management skills and reduce 30-day readmissions.

Learning Objectives

After completing this continuing education activity you will be able to:

  1. Describe the challenges associated with self-management of Type 2 diabetes mellitus (T2DM).
  2. Identify ways in which health coaching with adult learning principles are used to support self-management and lifestyle changes.
