Purpose of Activity
To provide information for case managers about the development of transitions-of-care (TOC) pathways for patients with pulmonary arterial hypertension (PAH) and idiopathic pulmonary fibrosis (IPF).Learning Objectives
After completing this continuing education activity you will be able to:
- Outline the development of the collaborative interdisciplinary TOC pathways and key foundations of knowledge about PAH and IPF.
- Recognize the epidemiology, signs and symptoms, and care gaps about PAH and IPF.
- Summarize the medication management, barriers to TOC, and role of case managers in transition planning for patients with PAH and IPF.
